Recommended actions to take
- Provide accessible services for people with serious mental illness (SMI) as required to meet their needs, clearly monitoring and addressing health outcomes.
- Ensure that systems are in place to engage and support attendance among those who are harder to reach (i.e. longer appointments, peer support opportunities etc).
- Ensure that care planning reflects the service users’ wider needs, including social care and mental health needs (using a care plan template such as the Bradford template).
