This page offers samples of work and supporting information from some of the primary care pilots we have been supporting across Bexley, Islington and Hammersmith & Fulham.
Below is breakdown of each pilot, with links to the resources created.
We supported Bexley CCG and neighbouring areas to carry out a number of projects that aimed to improve the treatment and management of local children and young people.
A project where local community pharmacists conducted opportunistic inhaler reviews of CYP with asthma aimed to make better use of this important resource and improve links between primary care staff across the locality.
The benefits of group consultations for children and young people were also trialled in Bexley and neighbouring areas. Three local practices took part, receiving training to deliver group consultations for young people with asthma and sickle cell anaemia.
A learning needs assessment was also conducted in Bexley to establish levels of knowledge and confidence among GPs and other practice staff in treating children and young people.
Hammersmith & Fulham
We supported Hammersmith & Fulham GP Federation to carry out a pilot that aimed to improve the treatment and management of local children and young people with asthma. This included the development of a professional and patient education event and testing of a prevalence finder for children with undiagnosed disease.
A learning needs assessment was also conducted in Hammersmith & Fulham to establish levels of knowledge and confidence among GPs and other practice staff in treating children and young people.
The posters below describe a programme that was developed in Hammersmith & Fulham to improve asthma in local CYP, increase integration across the area and support the development of GP leads.
We supported Islington CCG in relation to work on the development of integrated care networks. See the event materials: Tom’s story presentation, Tom’s story Part 1 and Part 2 (sound files) and Tom’s story narrative script
