Enhanced health in care homes

On this page you’ll find resources and information to support care homes and commissioners to improve the quality of health in care homes and patient flows across London.

Enabling people to live in their own home as long as possible is a key ambition for health and social care. For some people, a residential care setting will become their home and it is important that they remain part of the community and have access to health and social care that is tailored to meet their needs.

It is also important that when people can no longer live at home we provide the best level of care to those in a residential care setting. Our Enhanced Health in Care Homes Programme builds on the Enhanced Health in Care Homes Framework and focuses on sharing good practice across the capital.
