Enhanced primary care for care homes

Improving support from primary care into care homes is a key focus of the Enhanced Health in Care Homes Framework. Building on the work that happens locally, we aim to support care homes by sharing good practice and developing new models of working.

Medication optimisation in care homes

The National Medicines Optimisation Programme has made new funding available to recruit additional pharmacists and pharmacy technicians to support care homes in London.

Following the success of all applications received from each STP; all areas across London have recruited to the respective posts.

All pharmacists and pharmacy technicians are required to take part in a two year training programme lead by the Centre for Pharmacy Postgraduate Education (CPPE). CPPE aim to develop the skills and knowledge of participants, in order for them to continue as lead roles in this area of expertise once the programme has been completed.
