In February 2017, NHS England published a suite of urgent and emergency care interventions - Consolidated Channel Shift Model (CCSM) - designed for system leaders to respond to increasing demand. It will be of interest to commissioners and providers as they prepare for the increase in demand during winter.
This summary report has been shared with London Sustainablity and Transformation Planning Leads, Programme Directors, Chief Officers, Chief Finance Officers and Chairs. Interventions are grouped by the following categories:
- Delivering care in the community
- Reducing inappropriate admissions

- Reducing time spent in hospital
